Well, I got a bit paranoid, since 17% => I have a few Laz's running around. So I did a quick search. Didn't find the article you mentioned in Men's Health, but found this ancient article:

http://www.menweb.org/fdacondm.htm
Although condoms are generally expected to break less than 2 percent of the time--with more than half of the breakages occurring before ejaculation--real-life pregnancy rates over a year of condom use may be as high as 15 percent.
Inconsistent or incorrect use of condoms explains the discrepancy, according to Lillian Yin, director of the division in FDA that regulates condoms and other reproductive devices. One national survey of heterosexual adults with multiple sex partners found that only 17 percent used a condom every time they had sex.
"People say they use condoms," Yin says, "but do they use them each and every time and use them correctly? That's another ballgame. We hear it all the time--'We tried to use it, but ....'"
But what? Partner trust was the most-cited reason for not wearing condoms in a recent study sponsored by the National Institutes of Health. But be careful, CDC cautions, because even a trustworthy partner could unknowingly have a sexually transmitted disease.
The numbers match, so I wonder what Men's Health's sources are.

How do you incorrectly put a condom on?

i have no idea, but the expert in Laz's qoute even specifically said, "Incorrect use..."

no wonder there are so many stupid people out there. the ones that dont even have enough common sense to correctly put on a condom are the ones reproducing...
condoms break more often then you think
The only place I've ever had condoms tear on me is at the base, and I always noticed pretty quickly. I find it pretty hard to believe that a condom breaks easily anywhere toward the tip first (without assistance). That's the least strained part of the whole thing
(I mean....unless you're BR and have some crazy stainless steel toys involved)

My real suspicion is that when those surveys where answered, people who 'accidentally got pregnant even with a condom' just didn't want to fess up to the irresponsibility of not using a condom "that one time".

Haha, I think "incorrect use" is synonymous with the guy telling the chick "Yeah, I'm putting it on" but not actually doing so.

Hahhahha

Ok I think I am starting to understand this incorrect use issue now. Like hrmm I am about to cum let me do these last 7 thrusts without a condom and I will pull out.

I can remember a condom breaking on a few occasions. I have no idea why they broke but it wasn't at the base. It was more in the middle. I have a feeling it was from more of a prolonged sexual encounter. Or maybe the girl had some sort of pussy magic and knew how to break the condom.

Lol. I don't know. But girls believe some weird things when they're turned on.

I've only broken a condom once.

Since then, Trojans for life.

Or maybe I'm just hung like a bee, and have very little chance of breaking them :P

I've only had a condom break once and it was at the tip. I did feel something different, but I didn't realize what had happened until afterward.

But then I've only had sex 6 times in my life so 17 percent is about right.
