What does it mean when a girl has known you for a little while already, she has a boyfriend, but she's never mentioned him to you. Do you take this as an IOI? Chalk it up to memory lapse? Or maybe she just assumes you somehow already know?

You ignore it. If she wanted to tell you she would.
Zan Perrion has really good saying. "All women are my women." If you take this viewpoint then her boyfriend hardly matters because she is YOUR woman.
If she is really serious about him, if you make her uncomfortable, or if she thinks your hitting on her, then she will pipe up with it.

In what context do you know her?
Is she a co-worker.
A friend of a friend?
A girl you number closed at a bar?

Maybe it means absolutely nothing and she doesn't feel she has to, wants, or needs to share that aspect of her life with you.

I'm thinking of a co-worker specifically, though I am not trying to game her. I was just curious about this kind of situation in general, and if anyone had any theories about it.

Early in the conversation it means you showed too much interest. Late in the conversation means that she wants you to be aware that she has a BF and you shouldn't fall in love.

And if she doesn't mention her boyfriend at all, then you're not in a position to know her personal business. Even if you're flirting, if she had any intention of acting on it she would mention the boyfriend; otherwise, it's just harmless flirting.
If she doen't mention the boyfriend, I wouldn't take it as an IOI.
